Kindly submit the following documents needed for admission:

- Application Form
- Recommendation Form (to be filled out by previous teacher/guidance counselor)
Upload scanned jpeg files of the following documents:
- Birth Certificate (PSA)
- Baptismal Certificate
- Certificate of Good Moral
- Report Card (latest grading period)
Note: Please review all the information to ensure the completeness and accuracy before submission. Take note of your correct email address and contact information to ensure delivery of notification.

All applications and uploaded documents will undergo evaluation process and application verification will be done through a phone call interview.
Note: Upon qualification to Canossa School, applicant must submit the hard copy of the following documents:
- Photocopy of PSA Birth Certificate
- Baptismal Certificate
- 3 (1x1) photos.
